Founded in 2009, the Wyss Institute for Biologically Inspired Engineering at Harvard University is a world leader in biodesign engineering. For this exhibition, scientists at the Wyss Institute conceived of the theme of Biofuturism and selected works from the museum’s collection to describe the progression of ideas, objects, visions, and collaborations throughout history that culminated in this new approach to Design Science. Design terms can seem like jargon to non-designers, which can lead to miscommunication.
This supplement seeks to clarify its value and document lessons learned, while also distilling and demystifying design. We hope the supplement can act as an inspiration in building a shared vision of how design can advance impact in global health. Through a series of engagements in the Health Design Lab, students learn and apply Health Design Thinking methods to identify a real world healthcare challenge and design solutions that can have a real impact on people's lives.
Indeed, 7 of the studies included in this review reported literature reviews, and possibly expert consultation, as their only needs assessment steps, and none reported brainstorming. One way to overcome this tension is to view evidence as a set of design constraints in which needs assessment, brainstorming, ideation, and prototyping should occur. We believe that the work we do is profoundly important, and that it is our responsibility to continually improve the quality of healthcare to promote positive patient outcome. Data were collected on target users, health conditions, intervention, Design Thinking approach, study design or sample, and health outcomes. Studies were categorized as being successful (all outcomes improved), having mixed success (at least one outcome improved), or being not successful (no outcomes improved). Employing successful co-creation is exemplified in CyberRwanda,13 which was led by a multidisciplinary team of designers, public health experts, and evaluation specialists. From 2016–2019, the project deployed a youth-driven and youth-led design process with more than 600 Rwandan youth, caregivers, teachers, health care providers, and government stakeholders. Four studies directly compared interventions created with Design Thinking to interventions created with traditional methods.

As the numerous examples in this brief show, universal design has the potential to bridge the gap between basic human rights and higher human needs—for everyone. While roughly one-third of people 65 and older in the U.S. have some type of disability, two-thirds do not (US Census Bureau, 2015a).

Neonatal Intensive Care Units contain some of the most fragile and vulnerable patient populations. Many NICU babies have little ability to regulate their own bodily systems, heightening the importance of their immediate environment in their development. The purpose of this research study was to conduct a multiple method pre and post occupancy evaluation of an academic medical center’s Neonatal Intensive Care Unit (NICU). This study followed a mixed-method approach wherein surveying via questionnaires were used to evaluate user perceptions. Lighting measures and acoustic readings were used to support the questionnaire responses.

Universal design first appeared in the United States in the 1980s as a means to design for a wider range of users. A decade later, the Center for Universal Design (Story, Mueller, & Mace, 1997) developed a process for defining and evaluating universal design, referred to as the Seven Principles of Universal Design. Environmental cleanliness affects the patient experience, patient satisfaction, perceived service quality, and actual quality in terms of infection prevention.
The project engaged members to ensure the prototypes/pilots that emerged were sustainable and desirable from the perspectives of both advisory group members and men living with HIV.12 An advisory group provided valuable input at strategic points to allow changes in direction in real-time. An invitation to both designers and public health professionals to join forces more openly and more often to bring together the plurality of expertise within public health and the practical, people-centered, problem-solving approaches of design. In this webinar hear what happened when two of the industry’s leading healthcare design researchers teamed up with senior leaders in Project Planning and Nursing from Indiana University Health Adult Academic Health Center to take their design decision-making to the next level. They’ll share their highly-collaborative design and evaluation process along with the unique patient room design insights they uncovered. In order to create opportunities for social capital, many senior living and senior care facilities are partnering with schools and daycares to provide opportunities for intergenerational programming. Intergenerational learning—sharing skills, knowledge, and activities among young and old—can promote understanding, personal development, respect, and social capital (Femia, Zarit, Blair, Jarrott, & Bruno, 2008; See & Nicoladis, 2009).

The user/needs assessment could include contextual observation of users in the setting in which they would interact with the innovation, interviews, narrative accounts, and documentation from users, gathering extreme user/outlier stories or a review of existing literature and work (2,6). Prototyping included activities such as creating a series of low-fidelity and high-fidelity prototypes of the potential innovation and refining it multiple times through iterative cycles of feedback from end users, stakeholders, and experts. Testing the intervention with target users included implementing and testing the innovation while continuing to refine it on the basis of user feedback and data (1,2,4).
It is incumbent on healthcare futurists and leaders to rethink healthcare work environments. Creating spaces that not only support patient outcomes, but that support staff fulfillment, resilience and organizational commitment will be required for success as the healthcare industry seeks to sustain itself in the midst of high patient volumes and high operating expenses. Understanding the multifaceted impact of the built environment includes recognizing how acoustics, interruptions, break spaces, social support, and access to nature can be leveraged to support staff. Design Thinking is being used in varied health care settings and health conditions, and more studies are forthcoming.
An evidence-based design (EBD) process would support any or all of these approaches. While any individual approach may move us ahead of a traditional design process of user meetings, we can aspire to an EBD process where these three approaches meet, leveraging the benefits of each. Population health refers to not only the overall health of a population, but also the distribution of health. Ergonomic design studies human behavior and anatomy to make products easier to use.
